For Veterinarian Licenses

Veterinary Medicine Examination Application Package
This form is used by individuals that will graduate or have graduated from an American Veterinary Medical Association (AVMA) accredited school or university with a doctor of veterinary medicine degree. This form is also used by individuals who are foreign graduates and are enrolled or have completed the Educational Commission for Foreign Veterinary Graduates (ECFVG) program sponsored by the AVMA. Applicants must have successfully completed the North American Veterinary Licensing Examination (NAVLE) exam within the past five years.

Endorsement for Veterinary Medicine Examination Application Package
This form is used by individuals who have a current license in another state for at least three years or has successfully completed the national board exam. All endorsement applicants must have a Doctor of Veterinary Medicine (DVM) degree from an AVMA accredited school or university or an ECFVG certificate.

Registration of a Veterinary Premise Application Package
This form is used by individuals that want to open a permanent or mobile animal hospital or clinic. The applicant must submit the name of a licensed veterinarian who will be responsible for the management of the hospital or clinic.
